Complete our online registration form with your details. If necessary we can request their medical history from your previous vet.

Important - On registering with us, if your pet has visited another veterinary surgery for treatment, we will require their previous clinical history before your pet's first appointment here at our surgery. Please request this from your previous vet and ask for it to be emailed to haroldhill@hillparkvets.co.uk or elmpark@hillparkvets.co.uk
Alternatively you can inform us of their details so we can obtain the history for your pet.
